HR Support Ticket Form Structure
The form is divided into three main sections:
1. General Information: * Collects Full Name, Email Address, and Employee ID (if applicable).
2. Support Categories: Users must select one of the following to route their request:
Recruitment & Hiring: Application status, interviews, and job openings.
Onboarding & Training: New hire paperwork, IT setup, and orientation.
Payroll & Compensation: Paystubs, direct deposit, and tax forms.
Benefits & Wellness: Health insurance, PTO requests, and leaves of absence.
Workplace Support: Manager tools, policy questions, and workplace environment.
Other General HR Inquiries: Any miscellaneous HR issues.
3. Ticket Details & Attachments:
Requires a Subject Line and a detailed Description (with a warning not to include passwords or sensitive medical data).
Allows optional file uploads (PDF, DOCX, PNG, JPG up to 10MB) for items like medical certificates, receipts, or resumes.
Privacy & Confidentiality
Submitted information is treated with strict confidentiality.
Access is restricted solely to authorized HR personnel in compliance with company policies and local labor regulations.
